This was just a random idea I had while responding to a contribution from Miloš Stanković.
Perhaps, a way to oppose unhealthy fast-food chains would be to install protest art sculptures in the vicinity of these stores.
Sculptures of overweight humans could be placed next to sculptures of their equivalent weight in burgers for example, or a mountain of sugar can be made to illustrate how much sugar is consumed by a town every year simply by drinking soft drinks. Another option would be to have contrasting sculptures near each end of the drive-thru. At the entrance, you could depict happy, healthy children playing and at the exit, you could show the extreme effects of overindulgence.
The options, as with any idea for art, are obviously endless. That is not the focus of this idea. Rather, I see it as a viable option to oppose unhealthy fast-food chains. There is not much the companies would be able to do about it and it might just discourage some people from buying food from them.

Fast food chains would surely be very resistant towards this, they might find ways of how to ban such art, for example, arguing that in the vicinity of their establishments it can't be placed, they might find legal loopholes to justify this or employ other means to circumvent such initiatives. I think it might be wiser to use this method more indirectly, like placing the sculptures near the stands with fast food commercials in the city, maybe occasionally flying drones over fast food restaurants carrying a discouraging banner or a whole sculpture of an obese person, etc.

I believe sculptures of overweight people would not pass easily as they would be considered offensive for real overweight people. Instead of that, how about making sculptures of junk food items showing their true ingredients? It would look like a 3D infographic in the shape of a hamburger, a coke, a packet of French Fries. For example, 40% fat, 30% sugar, 20% you cannot pronounce it, 5% water, 5% we don’t know. Ideally, they would be made of the actual ingredients in the right proportions (as Milos Stankovic mentioned fat would look disgusting). But as that would probably be impractical, they could at least be made to look like those ingredients.

That's a great idea. Addressing weight directly is always going to be controversial. While I don't mind controversy, having something "cancelled" would defeat the purpose of raising awareness, so that is definitely something that needs to be considered.
Perhaps, considering information in articles like this, one would be able to create an even more shocking depiction of what people are putting into their bodies.

Ideally, the statues could be made out of actual human fat (or something resembling it) because it's absolutely disgusting and such a strong emotion might spur people into action.
We could also tie in the idea Darko Savic('s kid) had with the scratch-off cards, although not necessarily for pre-teens, and have a healthy human body to scratch off on one side. With the obese person to scratch off on the other side. Or just the obese person and one side and use the other side as an invitation for the exhibit.
